ADHERENCE SUPPORT INTERVENTIONS FOR HAART: CRITICAL ISSUES
ISSUES
Inconsistent adherence to highly active antiretroviral therapy (HAART)
can lead to medication resistance and transmission of mutant drug-resistant
HIV strains. Therefore, helping HIV patients to follow the complex medication
regimens demanded by HAART and remain on therapy is an urgent public health
priority. In response to the need for rapidly addressing medication difficulties,
adherence support programs have been implemented in clinical and community
settings, often in the absence of knowing whether the strategy is effective
for the target population. Many important questions about the effectiveness
of adherence support interventions remain unanswered, including: (1) what
intervention models work best with what populations and for how long;
(2) what are the intervention dose and threshold levels; (3) what is the
constellation of support services; (4) how useful is "readiness"/placebo
regimen training; (5) what is the impact of different types of providers;
and (6) what is the impact of service delivery system characteristics.
DESCRIPTION
A systematic evaluation of the effectiveness of theory-based adherence
support interventions in 12 AIDS centers is underway by the NY Academy
of Medicine to determine the type of interventions and care models needed
to help people initiate and maintain HAART.
CONCLUSION
Documenting and understanding the salient elements of adherence support
interventions will contribute to optimizing the effectiveness of the next
generation of programs to help people with HIV to take their medications
consistently.
